Electronic License application

WPCG provides online forms at no charge to contractors. They guide users to complete fields correctly and require an internet connection.

• Company admins get a portal view of all forms submitted by your company.
• You can export a PDF copy of each completed form from the portal.
• All accredited Clearance Issuers have access to the electronic forms via their portal.

After trialing the free forms, please confirm below and explain why your organisation still requires an Electronic Licence to use the forms inside your own system(s).

Terms of Use of WPCG Electronic Form Licence

This agreement sets out the terms and conditions under which the WPCG agrees to provide you with permission to convert the WPCG Clearance Form and Minor Work Control Checklists to an electronic format for use within your own systems.

1. Development Costs
   Any development costs now and into the future to implement and maintain the WPCG Clearance and Minor Work Control Checklists in your own system are at your company’s expense. As part of this agreement you will be required to keep the forms updated to latest versions.
2. Forms and Version Control
   Your company must have an internal company process that ensures that all technicians are given access to the current version of the forms. You are required to provide this information upon application, and at any time when requested during the term of the licence. Only the following forms may be converted under this agreement:
   • Work Clearance Form (v6.1 01/02/2025)
   • Minor Ground Disturbance Checklist (v5 15/01/2024)
   • Minor Work at Height Checklist (v4 01/02/2025)
   • Minor Hot Work Checklist (v4 01/02/2025)
3. Annual Licence Fee
   The annual licence fee applies upon approval of your application. An invoice will be issued once your application is received. Should your application not be approved, you will receive a full refund. No refunds will be issued once the application is approved and the forms have been provided.
4. Content of the forms
   Upon payment of licence fee you will be provided a set of the forms for you to have developed electronically. The content of the forms may not be altered in any way. An audit checklist will be provided to assist in development. Approval to commence using forms will be given in writing once you have provided pdf copy and displayed functionality of the forms on the platform being used. The initial license fee allows for one submission to be reviewed. Any further reviews required due to content errors will incur additional fee of $550incGST.
5. Use of the forms
   These forms may be used only in your own internal systems and by your employees. The licence is not transferable to subcontractors that you engage.
6. Annual Form Audit
   Your company will be required to provide annually three clearance forms and associated minimum control checklists for three different jobs completed by your technicians for auditing purposes. This must be provided along with the annual licence fee to maintain the licence
7. Amendments to these terms and conditions
   We reserve the right to amend these terms and conditions from time to time. Amendments will be effective immediately upon notification via e-mail from the WPCG. Your use of the service following such notification will represent an agreement by you to be bound by the terms and conditions as amended.
